چکیده

Objective: The present study aimed todetermine the time course of aminoglycoside induced serum creatinine (SCr) elevation and compare that in patients treated with amikacin (AK) and those treated with gentamicin (GM). Methodology: A one-year, non-interventional prospective study of patients with normal baseline renal function and were administered either GM or AK. The study was carried out at the internal medicine department of Al-Watani governmental hospital. Outcome of interest was the time course of serum creatinine elevation during the course of aminoglycoside therapy. Data were entered and analyzed using Statistical Package for Social Sciences (SPSS 16). Results: The study was performed in 94 patients, who had to be administered GM or AK by intravenous injections. In both groups, the significant rise in SCr was detected on the 4 day of therapy. However, GM induced up to 32% increase while AK induced up to 19.5% increase in SCr from baseline levels. In the course of AK treatment, patients on single dosing frequency and those < 65 years of age did not show a significant increase in SCr levels during the 6-day therapy. In the course of GM therapy, patients on single frequency dosing, younger patients and females showed a significant elevation in SCr on the 5 day while elderly, males and those on multiple dosing showed a significant elevation on the 4 day of therapy. However, all patient categories on GM therapy showed similar extent of SCr elevation. Conclusion: In patients with normal renal function, GM and AK showed similar time course but different extent of SCr elevation. Amikacin induced no SCr elevation when given in single dosing frequency or when given to younger patients. Such effects were not observed with GM.

Diabetes Mellitus Type 2 Is Not a Risk Factor for Aminoglycoside Induced Renal Injury

The aim of the study was to investigate the extent and time course of aminoglycoside (AG)-induced serum creatinine elevation in patients with and without type 2 diabetes mellitus (DM2). This prospective study included patients with and without DM2 who were administered AG parenterally. The outcome of interest was the extent and time course of serum creatinine elevation suggestive of renal injur...

Reno-protective effect of Rheum turkestanicum against gentamicin-induced nephrotoxicity

Objective(s): Gentamicin belongs to the family of aminoglycoside antibiotics and is a preferred drug in developing countries because of its low cost, availability, and potent effects against bacterial. However, gentamicin can induce nephrotoxicity. In this research, hydroalcoholic extract of Rheum turkestanicum was used against gentamicin- induced nephrotoxicity. Rheum turkestanicum is used aga...

Effects of Coenzyme Q10 on Hemoglobin A1C, Serum Urea and Creatinine in Alloxan-Induced Type 1 Diabetic Rats

Coenzyme Q 10 is a natural antioxidant and free radicals scavenger. In the present study, we examined effect of coenzyme Q 10 on hemoglobin A 1C , serum urea and creatinine in alloxan-induced Type 1 diabetic rats. Thirty Sprage-Dawley male rats were divided into three groups randomly; group one as control, group two diabetic untreatment, and group three treatments with coenzyme Q ...

Serum Creatinine Levels May Not Necessarily Reflect a True Renal Function to Adjust Amikacin Dose in Paraplegic Patient: A Case Report and Literature Review

To estimate glomerular filtration rate as an indicator of kidney function, clearance of endogenous creatinine is usually used despite its unreliability due to influence of several factors including age, gender, muscle mass and interferences within methods. Although there is emergence of better markers like serum cystatin-C, creatinine clearance continues to be used routinely as a marker of rena...

Elevated Vancomycin Trough Concentration: Increased Efficacy and/or Toxicity?

Vancomycin susceptibility of methicillin-resistant Staphylococcus aureus has been changed over time and its average minimum inhibitory concentration increased from 1.5 to 1.75 mg/L. A recently published guideline by the American Society of Health Pharmacist recommended a daily dose of 15-20 mg/kg every 8 to 12 hours of vancomycin to achieve a trough concentration between 15-20 mg/L for treatmen...
